What We Get Wrong About Male Skin

Male skin is roughly 25% thicker and ages on a different curve — why protocols built for female tissue miss, and what we do instead.

Men's skin is not just female skin with a shortened skincare routine.

It is biologically distinct. It ages by a different set of rules.

For years, aesthetic treatments were designed around female patients. When men started coming in, the same protocols were used with only minor adjustments. Depth, density, and skin structure were not fully accounted for.

The result was treatments that didn't go far enough to create real change. Results felt minimal, and most men were never given a clear explanation of why. Many left thinking aesthetic treatments just don't work for them.

They do. They just weren't designed with men in mind.

The Biology the Industry Ignored.

The clinical literature on male skin has existed for decades. This is not new science. What is new is a practice willing to act on it. Male skin is distinct from female skin in five measurable, clinically significant ways.

Structural Density. Male skin is approximately 25% thicker than female skin. Denser collagen architecture, greater tissue mass, and higher overall dermal volume mean that standard laser depth settings — calibrated for female tissue — frequently fail to reach the therapeutic layer in male patients. The device is working. The energy is landing in the wrong place.

Sebaceous Activity. Testosterone drives significantly higher sebum production. Larger pores, higher oil content, and a distinct pattern of textural irregularity characterize male skin — and respond poorly to treatments designed for low-sebum biology.

Vascular Architecture. Male facial skin has a denser vascular network, resulting in distinct patterns of redness, vascular irregularity, and rosacea-like presentation — typically at greater tissue depth than in female patients. Surface-targeting vascular treatments routinely miss the structures they are meant to address.

Follicle Structure. Coarser, deeper, and more densely distributed follicles change how laser energy disperses through male tissue. A wavelength calibrated for fine female facial hair is a different clinical tool than what male skin requires.

The Collagen Cliff. Through the thirties and into the early forties, testosterone actively supports collagen architecture. Male skin holds. Then testosterone declines. The scaffolding weakens. And decades of accumulated UV damage, oxidative stress, and inflammation that had been sitting beneath a dense dermis surface simultaneously. Men do not age gradually. They age similarly to a cliff. Stable through 42. Significantly changed by 46. The deterioration that appears sudden has been building structurally for a decade. The window to intervene — before visible change compounds — is the window most men spend doing nothing, because nobody told them the cliff was coming.

GentleMax Pro — Built for Depth. Right for Male Skin.

The GentleMax Pro by Candela operates across two distinct wavelengths: 755nm Alexandrite and 1064nm Nd:YAG. That dual capability isn't just another feature on a list. It's a way of making clinical decisions, and for male patients, it really changes what's possible.

755nm Alexandrite targets melanin preferentially. Exceptional for surface pigmentation, superficial sun damage, and fine vascular irregularities close to the skin's surface. Highly effective in the right clinical context.

1064nm Nd:YAG penetrates significantly deeper into the dermis. Less preferentially absorbed by melanin, safer and more effective over a broader range of skin tones — and critically, capable of reaching the structural tissue layers where male skin damage actually lives.

For male patients, the 1064nm Nd:YAG is the primary clinical instrument. Because male dermal density places the relevant tissue layers deeper than in female skin, shorter-wavelength devices, even excellent ones, frequently do not reach the treatment zone. The 1064nm does. That depth differential is the difference between a surface result and a structural one.

What GentleMax Pro Addresses in Male Patients.

At VIV Wellness, GentleMax Pro is applied across four specific presentations in male patients — each one showing a biological characteristic that standard treatment approaches underaddress.

Vascular Irregularities. Testosterone-influenced sebaceous activity creates a skin environment prone to vascular dilation, chronic redness, and rosacea-pattern presentation. In male patients, these vascular irregularities develop at greater tissue depth than in female patients. The 1064nm Nd:YAG delivers targeted energy to the vessel wall at the depth where it actually exists — not approximated from above by a surface-targeting alternative.

Deep Pigmentation and Sun Damage. Most men over 40 carry substantial UV damage accumulated in the mid-to-deep dermis across decades of outdoor exposure, typically without consistent sun protection. Topical treatments cannot reach this layer. Most surface-targeting lasers do not achieve clinical efficacy. The 1064nm wavelength does — and the improvement in overall skin tone, clarity, and evenness that follows is frequently the most significant visible outcome male patients experience.

Laxity and Textural Change in the Lower Face and Neck. The jawline, jowl, and anterior neck are the anatomical areas where male aging changes present first and are addressed last. By the time visible laxity appears at the surface, the structural deterioration driving it has been progressive for years. GentleMax Pro addresses tissue laxity at the level of origination — not at the surface presentation.

Hair Reduction with Follicle-Appropriate Precision. Male facial and body follicles are coarser, deeper, and more resistant to shorter-wavelength laser energy. The 1064nm Nd:YAG reaches the follicle base in male tissue to the required depth for effective, durable reduction, producing meaningfully better outcomes for beard shaping, body hair reduction, and the treatment of ingrown hair patterns than shorter-wavelength devices can reliably achieve.

The VIV Wellness Protocol for Male Patients.

The GentleMax Pro is the most capable tool we have for male skin. What it can't do is think. It can't assess where your collagen cliff is, what your hormonal environment looks like, or which layer of your dermis needs the energy most. That is what the clinical consultation is for.

At VIV Wellness, male patients receive protocols designed from the ground up for male biology — not adapted from a female template.

  • Structural Assessment Before Treatment. A 42-year-old approaching the collagen cliff and a 55-year-old past it present similarly on the surface. Clinically, they require different approaches. We assess a patient's stage of structural aging before selecting a device or applying any settings. The assessment drives the protocol. Not the other way around.
  • Individual Calibration. Default device settings are built for average tissue. Male skin is not representative of the datasets from which those defaults were derived. Every parameter, depth, energy, pulse duration, and treatment interval, is set for the individual patient on that day.
  • Functional Medicine Integration. The skin is the visible expression of what is happening internally. For male patients, the hormonal environment is a direct determinant of the skin aging trajectory — testosterone levels, cortisol load, and inflammatory markers all determine both the rate of structural decline and the tissue's capacity to respond to treatment. A male patient with unaddressed hormonal decline or chronic systemic inflammation will achieve less durable aesthetic results than one whose internal environment supports the external work being done. VIV Wellness integrates functional medicine assessment into male patient care for those who want to address the full picture — not just the surface one.
  • Treatment Sequencing. GentleMax Pro, paired with SkinPen Microneedling, addresses both vascular and pigmentation concerns, as well as structural collagen deficits — outcomes neither treatment achieves on its own. The order, interval, and combination of modalities are clinical decisions, not scheduling preferences.

The Timeline — And Why Now Matters.

If you are reading this at 35, the structural changes have already begun. Collagen synthesis has been declining since your late twenties. The UV damage from your twenties is sitting in your dermis, waiting to express itself. You cannot see most of it yet.

If you are 45, you are likely at or approaching the cliff. The changes you have noticed in the last two years are not random — they are the accumulated biological debt of the previous decade surfacing simultaneously. The window for structural prevention is narrowing.

If you are at 55, visible changes are present, and internal drivers are active. The clinical approach at this stage is not reversal. It is arrest, improvement, and building the environment — internally and externally — that prevents further acceleration.

In all three cases, the right time to act is now. Not because of vanity. Because biology does not pause while you decide.

Here Is the Honest Version of Why This Happened.

The aesthetic industry did not fail male patients because it lacked science. The biology has been documented for decades. It failed to reach male patients because they were not a prioritized market. Building male-specific protocols required investment. The return on that investment was not obvious. So it did not happen.

What happened instead was modification. Female protocols adjusted slightly — or not at all — and applied to male tissue. The results speak for themselves.

At VIV Wellness, the starting point has always been the patient in front of us. Not the protocol designed for someone else.

If you have had treatments before and walked away wondering why the results were not what you expected, you probably already sensed the answer. The treatment was not the problem. The protocol was.

You were not told that your skin ages differently. You were not told the treatments you received were calibrated for someone else's biology. You were not told that the decade before the cliff is the most important one.

You are being told now — and the door is open.
